Service Offerings:
Skilled Nursing Services: Comprehensive medical care including:
Clinical Assessments: General health, cardio, and pulmonary assessments.
Wound & Ostomy Care: Negative pressure wound therapy, incision care, and compression therapy.
Infusion Care: Antibiotics, hydration, and central line/PICC line maintenance.
Specialized Care: Ventilator monitoring, tracheostomy care, nephrostomy tube care, and G-tube/J-tube management.
Therapy Services: In-home rehabilitation provided by a multidisciplinary team including Physical Therapy, Occupational Therapy, and Speech Therapy, as well as support from Medical Social Workers.
Caregiver & Private Duty Services:
Personal Care: Assistance with Activities of Daily Living (ADLs), meal preparation, and light housekeeping.
Private Duty Nursing: Skilled nursing shifts or visits tailored to individual patient needs.
Respite Care: Temporary relief for family caregivers, available from 2 to 24 hours a day.
Community & Training Services: The agency also serves as a community resource hub, providing Direct Care Worker (DCW) training, Article 9 training, fingerprinting, and notary services.
